The Division of Services to the Disabled is the local issuing agent for the New York State Special Vehicle Identification Permit. Permanent or Temporary Permits are issued to individuals with severe mobility impairments. This special permit allows the bearer to park his/her vehicle at posted disabled parking spaces in certain off-street public parking spaces or wherever similar spaces are established on public roads or highways.
There are two sections on the permit application; one for the applicant and one for the physician to complete with medical information. If you would like an application, call the Division of Services to the Disabled at (631) 224-5335 (Voice) or (631) 224-5397 (TTY)
